How to use this list: Each link opens VA’s own blank form (a PDF hosted on benefits.va.gov). A DBQ is one way to give VA the medical findings it needs; a provider’s narrative letter or treatment records can also serve as evidence. Which form fits depends on the condition being claimed, so match the form to the diagnosis, not the other way around. Conditions marked “Rating criteria” link to the VA rating criteria page.

This is a convenience copy of VA’s public DBQ list so you don’t have to hunt for forms. VA updates its forms from time to time, so for the newest version of any form, check VA’s official public DBQ page. List last checked October 2026.
